K&W Underground partners with carefully selected contractors for various phases of telecommunication installation.

If you are a contractor that would like to be considered for future projects, please complete the Subcontractor Prequalification Questionnaire.

Once the questionnaire is submitted one of our management team will be in contact.

REQUIREMENTS

1. Completed and Submitted Subcontractor Prequalification Questionnaire.

2. Provide certificate of insurance that meet our requirements as stated in our subcontract agreement.

3. Provide a W-9 with updated information annually.

4. Provide a copy of current Experience Modification Rate (EMR) on insurance letterhead, annually.

5. All work performed in accordance with K&W Underground, industry, regulatory and local standard quality, practices and safety requirements.

6. An executed Subcontractor Agreement.
